Dear Patient:

Weight loss and healthy diet to lose body fat, for obese and overweight patients in my internal medicine practice is controlled in terms of food, exercise, calories, weight watching and medicine approved by FDA. Our medically supervised program will provide you with the infrastructure to accomplish your weight loss goals quickly and safely. The first phase is the acute weight loss phase, which lasts until the desired weight loss is attained. The second phase is the transitional phase during which you will discontinue your medication, and you will be introduce to a higher caloric diet and increased exercise regimen. The third phase is the long-term maintenance phase, which will give you the support and guidance to remain at your ideal weight.

You will be instructed on our Keys to Success Program, which is proven to lower your blood insulin level. This process increases the body´s ability to burn fat, and creates the foundation of maintaining weight loss. A comprehensive blood panel, EKG, and your weight and body mass index measurement will be performed. You will also be provided with a basic exercise regimen, which is a very important component to a successful weight-loss program. A copy of the Weight-Loss Consumer Bill of Rights will be provided in your information packet.

I will evaluate your medical and weight history, and make the appropriate recommendation for your individualized program. You will be prescribed an FDA-approved appetite suppressant, which has been proven to be safe and effective for many years. Our program also utilizes supplements and injections that enhance your desired weight loss outcome.

Our program requires a weekly scheduled visit to assess your progress. Any indicated changes will be made if needed. We take great pride in our program, and thank you for your confidence and trust in us.
